Visit Document a Charge and complete the secure multi-step intake form. You'll provide:
Filing is completely free for consumers. You'll create a free CPI account first - it's how you track your case and post updates (refunded, denied, or ignored) as it progresses.
Immediately upon submission, CPI generates a unique Reference Number in the format:
CPI-YYYYMMDD-XXXXXX
CPI staff review submitted materials within 2-3 business days. Status updates are reflected in real time. Possible statuses:
Any party - merchant, bank, attorney, processor, or consumer - can visit the Verification Portal, enter the Reference Number, and select their identity type to see case status and basic details.
Consumer personal data is protected. Third parties see only the reference number, filing date, case status, merchant name, and a sanitized case summary.
For legal proceedings, attorneys and consumers may request an Expert Statement or Testimony. This requires a documented CPI Reference Number and service payment.
